I've not had issues with comment spam with the current config, but a friend using Wordpress was getting them even with Akismet. He's using reCAPTCHA, but some spam has still managed to get through. I like the idea of OpenID, but haven't tried it on my site yet. There was an interesting article recently about how timbl restricts comments. You have to use OpenID and be linked to him via FOAF.
