This patch solves the issue (I tested it with VirtualBox).

Haven't tried Opera, but in 1.3.6, Virtualbox 3.x (also uses Qt) appears
with the titlebar above the workspace top. 
I'm using Ubuntu 10.04, Icewm 1.3.6, and virtualbox-3.2 (virtualbox-ose
version 3.1`has same issue).
