Hello Cathy, That is odd. There should always be one called Standard Phone Form. I don't think that one can be edited. Assuming the folders still exist for all the fields he wants, he can go back into the PhoneBook menu, Change PhoneBook Form and either edit one that shows up or create a new one from scratch. It is fairly easy to do.
