If you are using 32 bit windows, then I am afraid it does no work reliably there.  No fix. 
 If 64 bit, I cannot explain.  I  use icons in both windows 10 and 7 without issue.  Do make sure you are referring directly to exe or ico file, not to shortcut.  There are unfixable (AFAIK) issues with icons and shortcuts to 64 bit programs resulting from fact that PowerPro itself is 32 bit..
 

 

 If neither 32 bit nor shortcut issue, perhaps it is some aspect of your particular configuration.
 

 To test, try making a copy of default.pcf, switching to the copy, then adding context menu command list.  
 Do icons work?  If so, it is some aspect of your config which you will need to isolate before  I can investigate.
