WTL::CCommandBarCtrl, ActiveX and WinForms

Hi,
I have an ATL ActiveX composite control that makes use of
WTL::CCommandBarCtrl internally and when hosted in the ActiveX test
container or VB6 everything is working fine... however when I try to use it
in a windows forms application any context menu or chevron menu generated by
the CCommandBarCtrl fails.
I'm seeing an assert fail in CCommandBarCtrlImpl::OnInitMenuPopup for bRet =
menuPopup.SetMenuItemInfo(i, TRUE, &mii);
which occurs for any context menu and an internal exception within .NET for
any chevron menu.
I'm cant see any thing wrong with parameters to the SetMenuItemInfo() call
which could cause it to fail.
Thought I'd ask for suggestions before digging into this too much further as
CCommandBar is a pain to debug/follow at the best of times.
