I've been using a Therm-a-Rest Haven 20 deg. down blanket for the last two seasons with an Exped Synmat UL7 sleeping pad.  Works great, quite comfortable down to 30 deg. after that I put on a baselayer.  My wife customized it for me, removing the straps, most of the elastic, and sewing the face hole closed.  This allows it to be used as a blanket with a footbox without being strapped in.  

You might also check out Warbonnetoutdoors, they also make down blankets with a foot box.
