Well, back in the Plain Vanilla days, if I remember correctly:

	1 No.
	2 Yes.

	3 No
		Because the text was done differently.

Furthermore, correction to a particular version was made to that version
and a new revision was released.

Today, the situation is somewhat more complex as the amount of information –
formatting is greater. So 2, would be the way to go, also.

Yet, if one just corrected the original submission( no idea how that could be done)
then just a revision bump should be done. 

As to the credit line, just use corrections submitted by e.g BB.
This would not imply a direct interaction between the original
submitter and the corrector.
