I implemented this myself back in the dark ages. it was easy except  
when the email to the user bounced or when the file was too large.  
You can also send other formats (like HTML) when you zip the files  
and send the zip. The benefit was that you could be sure the customer  
received the file (no partial downloads) and it was somewhat easier  
for the customer to track, because they could tell if they go the  
email with the book attached. I also implemented a system where if  
someone did not get their book, I could enter their email address and  
have the book resent. That was sometimes necessary.

This is the most common tactic.

Never seen or hear of this. Not even sure how you'd really do it.

Lots of people do this in addition to #2. Just because Amazon sells  
your ebook, doesn't mean that your web site shouldn't also sell it.
