As Mary said, it really does depend. I know a couple of people who
ghostwrite, on occasion. On a fiction work, they normally get some
sort of outline or story notes to work from. On non-fiction, they may
get that or they may get interview time, even online time to do it.
Either way, if the ghost writer encounters a problem, there's some
manner of asking questions for clarification later. I believe both of
them were paid flat fees for their work. How much depends on the
expectations for the book sales. Neither of them made nearly what OJ's
ghost writer was reported to have been offered, for instance. When the
draft is done, it goes to the "author of name," and the nits come
back. Hopefully, they are minor nits. If not... Well, let's say I know
one case where the ghost writer and "author of name" needed to have
the editor sit in as mediator on some technical problems the author
was creating. Let's just say there was a reason she needed a ghost
writer. Grinning...
