How detailed do you want to get and how many pages do you want people to
wade through? Your average ebook notes the publishing house, author, one of
the editors...sometimes two (and most of my houses use 2-4 editors/proofers
on every book), cover artist... Okay...now, SOME publishing houses
additionally have an entire marketing team that do things like blurbs and
such, formatters that aren't already named elsewhere, beta
readers/acquisitions readers that work for the publishing house... Some
projects additionally have ghost writers, guest writers (for an
introduction/foreword), contributors, and so forth... Some of these people
want to be named (or you want them to be named), and some you/they don't.
It's not as straight forward as it seems, and I suspect that it isn't done,
because it's considered a waste of space and an annoyance for the reader.
Heck, how many of us watch the full credits on a movie or TV show, if we
don't think there's a surprise in there somewhere?

OTOH, I've noticed some movies going sparse recently. I watched a movie the
other night that DIDN'T name the songs and performers for them in the end
credits. What the heck is that? I found the information I was looking for
online, but I always thought THAT was standard.
