Copyright page help!

Hi all,

My apologies if this has been covered before. I have a quick question:

Can someone here either tell me or point me to an online source that gives the REAL deal about what information is required on a copyright page for an ebook? I've seen conflicting info online as to what is actually required. Ironically, it appears that many people just pick up a book and, well, copy the copyright page. (?!) What I'd love is some sort of sample text that I know I'm free to use and tweak, and that I know satisfies any legal requirements for the copyright page.
