Copyright *is*.

So any copyright page is information that the publisher/author wants to 
make clear. There is no perfect or standard page. There are conventions, 
however, which is I think what you are asking about.

The © symbol is always good, the date can be required or a matter of 
choice, depending on national or local variations of law. Other things 
are choices. The EU has some variations.

One of the best starting points is the Smashwords copyright layout. But 
see some others and see what you want to put there. There are some 
differences, but the purpose and content are similar to print books.

The one thing to think through for ebooks is how strongly you want to 
remind people that however easy it may be, making copies of copyright 
materials is against the law; that law may be mostly civil at the 
moment, but it could become criminal.

There are only so many ways of putting a copyright page together, and a 
lot is factual bits and pieces like ISBN and other unique identifiers, 
whose copyrights are involved and so on. Write your own, but don't think 
it has to be much different.
