MLA citation question for h-film posting

A graduate student has posed this question:
I'm working on a paper in which I'm citing an article from a DVD insert booklet (Cocteau's press notes from the American premiere of "Belle et la Bete" found in the Criterion DVD) and I'm not entirely certain how to include that on a bibliography. I haven't been able to find a close match on this.
Ideas?
