I had a similar quandary last spring.  My student and I solved it like this: Cite the pamphlet insert as if a chapter in an edited volume, listing it as "Pamphlet Insert" instead of using page numbers, and treat the DVD as the volume.  It may not be the official answer, but it worked for us.  It has the advantage of being adaptable to almost any citation format.  I hope this helps.

Having checked out Amazon and the Criterion site, I would suggest something like this:
