I hear what you're saying about the transition to v3 from v2.
I too found it somewhat painful, but also surprised that I was
able to get it done in a reasonable amount of time.  I was
able to reproduce and improve upon everything I was doing in
v2.

Of course I use gnus and not mh-e so my experiences were
likely very different.

The issue of having .bbdb polluted with auto-captures is a
real one.  I have a couple of workarounds which definitely
don't apply to everyone.

One is simple, and also solves the mail sync problem to a
degree.  I leave all mail on my IMAP server and do spam
filtering there.  Anything in spam is excluded from .bbdb
capture.  Same for newsgroups.  No addresses are captured
unless I actually reply to them.  I make good use of
bbdb-accept-message-alist and bbdb-ignore-message-alist, as
well as some custom code which refuses to add addresses to
.bbdb if there are too many of them in the To: and CC: fields.
It's not perfect, and I do ingest some junk addresses, but it
works to a pretty reasonable extent.

Of course with mail left on the server it is automatically the
same on all devices.  Of course this means slower operation
and the need for an internet connection so nothing is truly
free.

.bbdb is synced with an rsync arrangement.

BBDB does require some effort but I've found what it gives me
in return is quite a lot.
