I tried to reproduce this with Emacs 21.1 and the latest from CVS but 
it work for me.

Definitely looks like all elements are listed. It is also odd that they 
are in upper case. It is the XML DTD isn't it?

I tried with DocBook XML DTD V4.1.2 and the elements have lower case 
names. Is psgml picking up the correct DTD?
