Strange behaviour

Can anyone explain, when I execute sgml-insert-element within the address
node, I get prompted for the firstname, but when I attempt to generate the
HTML, the parser says that no templates match firstname?

I am using 1.48 XSL of DocBook.

<address>
    <firstname></firstname>
    <street></street>
    <city>Fort
Lauderdale,</city>&nbsp;<state>FL</state>&nbsp;<postcode>33309</postcode>
    <country>USA</country>
</address>
