H-to-H CC with the Japanese Question

We have two H-to-H CCs between the Japanese and the Chinese.  The Japanese squads are 1st line in one CC and Elite in the other CC; all Chinese squads are Elite.  Both CCs are the result of a multi-hex Japanese Banzai charge and are occurring in different building Locations.  In one CC, a Banzai squad moved into the Location during the MPh locking those units in H-to-H CC (per G1.64).  That CC was reinforced in the APh by another unit (from the same Banzai).  Since this unit entered a building Location during the APh that was not in melee yet, Ambush is possible.  The Chinese Ambush the Lax Japanese.

Question 1:  Who is ambushed?  Just the squad that entered during the APh? Both units?  Does it matter if the Chinese attack the Japanese squads separately?

Question 2:  Since the Japanese were Ambushed, is the CC no longer H-to-H?

In the other CC there is no Ambush possible because the Japanese units entered during the MPh; the CC is H-to-H.  G1.64 says "Each Japanese H-to-H CC attack receives an extra -1 DRM ...".  The A11.11 CCT chart on my older Chapter A divider as well as the equivalent chart on VASL says "by unpinned ... Japanese (G1.64) vs Infantry/Cavalry in HtH CC (unless Ambushed) ......... -1".  I have found (and my opponent has) a newer Chapter A divider that says "...... -2" for the Japanese in HtH CC.

Question 3:  What are the CC DRMs for the Japanese for a leaderless, un-ambushed, HtH attack?
