In my experience, what gets smart people in high-level trouble is either designing the wrong thing (or for the wrong target), or over-designing, which leads to an overly rigid, verbose or cumbersome implementation. Half of the time, though, problems aren't technical at all, but more of the "let's put ten elephants into one phone booth by next Thursday" kind.
