Thanks, Bert. I will wait for your other comments and do another round
to fix the nits. It would be great if other folks can have a fresh look
meantime.

I have one question about the comments on references in the last part of
your mail. What was the tool you used? They seem to be machine
generated, and mostly derived from formatting. I will try to fix the
formatting but I am not sure I grasp the intent for many of them, and
then some other seem false alarms.
