It's labeled "Additional information (if necessary):" in the online form, where it's part of the "Intended Usage" field:

I believe an expert would've asked us to add that. I say this because I remember Ned asking us to add the "Additional notes (optional):" field that immediately follows the encoding consideration radio button options. Looking at archive.org, the intended usage comment field (usually left blank) has been part of the web form since at least 2000:

My guess would be that IANA would've only added the initial submitter name/email fields, which let people submit a request without listing themselves as the public contact.
