Is this problem occurring every time you plug the adapter in? Or is it
sporadic?

It's unlikely (but still a possibility) that you have a (physically)
dodgy board. This is what happened with me (the errors would appear at
'random' times during use). I thought it was the UHCI and just lived
with it for a while until I realised that if I applied pressure to the
connector in a certain way and didn't move the adapter at all,
everything would be fine (no such errors). I've since replaced it and
never unplug the little USB extension cable from its connector (no
problems).
