This ID is present in at76c505-rfmd2958.c in the version 0.13.  So, the
driver should be at76c505-rfmd2958.

That's strange.  12fd:1001 has always been in at76c505-rfmd2958, not in
at76c505-rfmd.

Better upgrade to version 0.13 or to the current CVS version of the
