Since the driver cannot interpret the EEPROM, it is assuming that this
is a multiple port board where the subsequent ports do not have an
EEPROM.  The primary fix is to use a driver that has the correct EEPROM
interpretation.

Does 'tulip-diag' correctly read the EEPROM information?

What is the kernel message when you load the module but do not start the
interface?
Can you eject and re-insert the card, and get the proper MAC address
reported in the kernel messages?
Can you start the interface with 'ifconfig eth0 <static IP address>'?
