It's not a multiport card, unless I'm misunderstanding what you mean.
It's only got 1 ethernet jack.

'tulip-diag' cannot read the EEPROM in either the 2.2 kernel or the 2.4.
The 2.2 kernel can somehow figure out the MAC address, while the 2.4
kernel cannot.  From this, I figure the card is no good and will try and
get a new one.  It just seems odd that it works, but has an identity
crisis.

Re-inserting the card gives it a new MAC address.  It starts at
00:4C:69:6E:75:79 and increases by 1 each time it is re-insterted, ie
00:4C:69:6E:75:7A is the next one.
