You certainly missed some. Did you notice the high number of reports 
of errors whenever somebody tried to force some media setting ?

mii-diag -F used to work.

Please note that initialization with 0x204 as you did disables MII.  
mii-diag requires a properly working MII, so the two might happen in 
different ways.
