Initially I had the same problems as you, but somehow those problems
went away.  FailSafe isn't simple to install as a package like
heartbeat;  but once you get a hang of it, installation is more
straightforward.

Make sure that you have FailSafe 1.0.4...either get it the source or
binaries from Suse(or grab latest cvs).  I recommend just grabbing the
source and compiling it if you don't use Suse(like me:)).

If you are replacing FailSafe with whatever version, make sure that cdbd
is really dead.  Otherwise I find that cdbreinit doesn't really do its
job properly.  Sometimes killing cdbd is nearly impossible with killall
-KILL cdbd, where all other base-level daemons are already dead.  The
fs_cluster script attempts to kill the base failsafe processes, but the
fsinstall script doesn't verify this and goes blindly ahead trying to
init the cdb.  If this is your case, disable fs_cluster init script,
reboot, enable fs_cluster and run fsinstall.

Using this process works for me, as I don't get those cdb problems
anymore:)  Hope it helps you too.
