Removing /var/lib/failsafe/cdb and re-running cdbreinit and cbd-create-std-nodes (or something like that) has got things going again.

As for documentation, if i can get a trivial setup going, i'll try to document the basics.

No idea. Potentially your cluster is not "online" but in "Unknown" status.

Well, what do your crsd logfiles say? You don't give enough information I am
afraid.
