There is a conflict within, there are two competing drivers namely the
snd_hda one vs slamr.  The one loaded is the slamr one :(, which does
not server the modem.  From scanModem output:

 An ALSA (Advanced Linux Sound Architecture) modem driver:  snd-intel8x0m
 provides Low Level support enabling contact with the modem hardware.
 For all BUT Conexant chip soft modems (using hsfmodem software)
 complementary High Level support is through a Smartlink utility:  slmodemd

 Download from http://linmodems.technion.ac.il/packages/smartlink/
 the package SLMODEMD_gcc4.4_alsa1.0.23.tar.gz having a compiled
slmodemd. Unpack under Linux with:
       $ tar zxf SLMODEMD_gcc4.4_alsa1.0.23.tar.gz
 and read instructions therein. But briefly, the modem is setup with command:
       sudo slmodemd -c YOUR_COUNTRY --alsa modem:1
 reporting dynamic creation of ports:
       /dev/ttySL0 --> /dev/pts/N   , with N some number
 Read DOCs/Smartlink.txt and Modem/DOCs/YourSystem.txt for follow
through guidance.

There is a program called sl-modem-daemon and sl-modem-?  search for
them, but they apparently should be loaded.  You may try
$ sudo modprobe -r slamr
and
$ sudo modproe -r snd_intel8x0m
reload  last one
$ sudo modprobe  snd_intel8x0m
and then try
$ sudo slmodemd -c USA --alsa modem:1

leave it running and from another terminal attempt to connect via wvdial

$ sudo wvdial

and hopefully this time it will connect.  Let us know if this makes
the modem work.
