Occasionally an obvious suggestion turns out to be helpful, so just in
case: are you running the scan as root? As normal user, I get:

$ /sbin/iwlist wlan0 scan
wlan0     No scan results
[and syslog says "p80211req_dorequest: wlan0: only dot11req_mibget
allowed for non-root"]

As root:

# iwlist wlan0 scan
wlan0     Scan completed :
          Cell 01 - Address: blah
                    ESSID:"blah"
                    Mode:Master
                    Encryption key:on
                    Channel:4
                    Quality:29/92  Signal level:-62 dBm  Noise
level:-91 dBm

This is with lwlan 0.2.3, D-link DWL-122, Fedora Core 4.
