pam failures revisited

This is old stuff, but I can't actually find the
information anywhere.  With Debian Sarge released,
people have started to run the 2.0 tests.  There
have showed some pam failures, we should perhaps
backport the "resetup accounts" change to lsb-test-pam
that just went into the 3.0 runtime.

So what causes these problems - does anyone remember?
I actually thought I'd seen them in a 3.0 run recently
as well.
