Help needed with OEM unsupported card, identical to working card

Gentlemen,
I have been looking at docs since months (not full time) to find out how
I can use an OEM (Apollo) modem card identical to the ActionTec FM560LKI
and Fiberline-FM560. Those are known to be supported.
My problem is probably stupid and due to lack of sufficient PCMCIA
experience.
The manfid should be 0x0200 0x0001 . Here is the logged message when I
plug it in:
