If that doesn't solve it (It always has, for any of the pads I've added!), then you'll need to look at adding some printk() debug to it (check that the rats nest of #includes don't helpfully undefine printk - it's always a pain!), and debug a bit further. We'd appreciate you sharing the output of lsusb -vv from linux with this pad connected, so we can add this mysterious pad both to cromwell and the linux kernel xpad driver, too.
