SCSI Error from System Log

Good day,

First, here are some info about my system:
-OS: RHEL 3 ES
-Kernel: 2.4.21-4.ELsmp
-Machine: IBM xSeries 235

The problem is my system log keep mentioning this error:

So guys, do you have any idea on what's happening? Does the hard disk is
having a problem? Or is the problem related with mainboard or storage
controller?
