FYI efi.h is shipped by the package gnu-efi in Slackware version 14.1.

You could build this package,... But then I'm not sure that the kernel
you use in a former Slackware version be properly configured for
EFI anyway ;)

So my (not requested) advice is: upgrade Slackware ASAP.
