Hmmm...would be nice but sounds like a fair amount of work.
 Alternatively, I think putting the top-level of the TOC
into a box on the left or right side shouldn't be too hard.

All I can think is we get rid of the opening text, as the
TOC is both on high on the left hand bar and the second
link off the main text area.

Agreed.  Probably an implementation problem again -- I
don't know if this is possible in TWiki without some coding
work.
