Have you told Blat what to use for username and password for your Exchange server? Sending emails within your domain seems permitted without authentication, from your description, but sending emails to external destinations might require a username and password. Check with the Exchange administrator for what is required.
If possible, add "-debug -log blat.log" to your command line to capture the dialogue between Blat and your Exchange server. That log will give hints to how to solve this issue.

Looking for a solution.
