While I'm not in the same boat as Mr. Klorese, it's becoming common, and
I dare say commonplace, that ISPs will only allow outgoing mail when the
sender matches either an AUTH'd user (via SMTP auth, pop-before-smtp,
etc...) or an email account that the ISP recognizes as one of its own.

I don't know how mutt does it, but most other MUAs I've used (Sylpheed
and Pine being two I'm quite familiar with) have different settings for
different accounts, and one of those settings is logically the smtp
server.  Indeed, most MUAs allow setting either an smtp server or the
path to a sendmail-compatible program which does the message submission
on behalf of the MUA.
