As you say, #3040 ( https://code.exim.org/exim/exim/issues/3040 ) code did get involved. That being so, in theory we should have not told the sender that we have accepted the message (thus, responsibility for it should remain with the sending system). However, I'm not convinced that the #3040 ( https://code.exim.org/exim/exim/issues/3040 ) coding goes as far as transmitting a temp-reject (which would be preferred). What did the sending system log? I'd guess something indicating a timeout or a conn-drop after data phase.
