Moderation request only going to one moderator.

I have a list using ezmlm-idx-std-0.53.400-10, which I know is old.

The list was configured with the following flags:

	F:-aBCDeFGHiJKlmNopQrsTUVWXYZ

These allow me to run a list where only moderators can post, and messages themselves are moderated (to provide a basic release sanity-checking capability).

Originally, only one moderator was configured - the address messages were sent from, and everything has been working fine for many years.

I've recently been asked to add additional moderators. 
Using:
	ezmlm-sub /list/mod [email protected]

Checking with ezmlm-list, I see my additional moderators. My address is one of them.

However, when sending a message to the list, the following is logged by the qmail log:

Why is the moderation request, which this is since it arrives in my inbox, only being sent to myself, and not all moderators as the documentation suggests it should?
