That's a pretty bad failure, and would cause duplicate delivery problems
that could be very hard to debug.

I must not have been clear.  ezmlm does *NOT* invoke qmail-inject.  It
invokes qmail-queue which does no message modification of any kind
(except for prepending a Received: header stamp).
