Obviously you have figured out way more about the pine source than I have,
but just FYI, I'm not sure if you knew what Steve meant.

You can turn on the expose-hidden-config pref, and then you can see more
configuration options, one of them being save-aggregates-copy-sequence.

The help for that feature says:
However, many IMAP servers (including the UW IMAP server) do not preserve the
order of messages when this optimization is applied.

This isn't your issue since you didn't have it set, but it is one thing
that could affect the order.
