I didn't interpret it that way.  Does anyone have a reference to the
original statement by DJB?

You're half right.  netqmail distributes an unmodified
qmail-1.03.tar.gz and patches and separate files within the netqmail
tarball.  But another file in that tarball is a script which does the
patching for you.
