Sure. If I send you an email, you shouldn't get a confirmation request
when you reply. But if my incoming filters are set up to ask for
confirmation, and my outgoing filters don't auto-whitelist (possibly
because outgoing mail isn't being filtered by TMDA), you will get a
confirmation request. I think this is probably a somewhat common
misconfiguration, because incoming mail and outgoing mail have to be
configured separately. Incoming mail may simply use .forward, but
outgoing mail needs either TMDA's sendmail replacement script (which
isn't a supported sending method in Thunderbird, for example) or
tmda-ofmipd, which has to be set up as a server and requires
configuration itself, plus configuration of all mail client applications
to make them use the tmda-ofmipd server instead of the "real" SMTP
server. This is all a bit complicated, and makes it all the more likely
that new users will not set it up correctly.

It looks like 99 is a qmail-specific code, so perhaps you shouldn't get
it with postfix. How did you set MAIL_TRANSFER_AGENT? It looks like it
can happen when -p or --print is passed to tmda-filter also, but I'm not
sure why that would happen.
