TMDA , Qmail - Mail not consistently released after verification

Testing, post CONFIRM, on some accounts the messages are not released. The test file is seen in the pending directory but there is no addition to the whitelist. Following the same installation steps, some accounts work like a charm and others DON'T! ??? What could be the possible problem, permissions across all accounts seems consistent too Thanks!
