There's not much information to go on here. Is this the complete
incoming log? If so, it looks like TMDA is never seeing a confirmation
message. I'd check that it's going to the address you think it is, and
check the MTA's logs to verify that it is being received and to see
how it's being handled. If TMDA never sees it, it obviously won't
handle it.
