New installation question

I have just installed vqadmin however I am experiencing a slight problem. I
get authenticated via the htaccess and can go into each of the options on
the Main Menu page, however if I try to search or add or delete anything it
returns me to the vqadmin.cgi and the Authentication Failed Username unknown
error. This happens regardless which menu option I try to use. I followed
the install instructions and the acl setup instructions, is there something
I completely missed in the configuration?
