I don't know how class would enter into it, other than I think it's
fine for people with the money to purchase whatever procedures they
want, cost/benefit be damned.

Oh, but I think they are. The price I pay for any given procedure
isn't completely determined by what that procedure actually costs. It
also has a fair amount of overhead built it, to cover people who don't
pay and other indeterminables.
