gpxe-0.5 cvs sync

I've just synchronised the gpxe-0.5 tree to the latest changes from 
etherboot-5.4, and tagged etherboot-5.4 with GPXE_0_5_MERGE.  I've done 
the best I could with the merges, but there's bound to be something 
broken.
