In snmpd.conf:

extend someString /usr/bin/echo MyMarkString

and then, after an agent restart

snmpwalk localhost nsExtensions

Note that it, admittedly, is rather limited in use.

Another thing that is worth mentioning is that nsExtensions ain't part of the
accessible mib space with a default configured Net-SNMP.

It would be helpful if you mentioned which part of the documentation you
found unhelpful and what you wish to see instead.
